Can you get semaglutide online in North Carolina?

Does insurance cover semaglutide in North Carolina in 2026?
For weight loss alone, sometimes. Wegovy is NC Medicaid’s preferred weight-management drug, covered with prior authorization. North Carolina’s state employee plan doesn’t cover GLP-1s for weight loss. With Medicare Part D, the GLP-1 Bridge can bring the Wegovy pen or pill to $50 a month.
NC Medicaid: with criteria
Yes, with prior authorization. Wegovy is NC Medicaid’s preferred weight-management drug, covered from age 12. It’s also covered for heart risk and MASH liver disease. A 2026 state law asks for a plan letting Medicaid health plans manage weight-loss GLP-1 use, starting no earlier than July 1, 2027.

No. The State Health Plan stopped covering GLP-1s for weight loss on April 1, 2024, but still covers them for diabetes; Ozempic is on its drug list with prior authorization. Wegovy isn’t listed on the 2026 formulary updated Oct. 1, 2026.

Job-based and marketplace plans: it depends on your plan
Coverage depends on your employer’s plan. Ask HR or the plan: Are Zepbound and Wegovy covered for weight loss? Is prior authorization required? Which BMI or health conditions qualify? If your plan is self-funded, North Carolina’s state external review won’t apply.
If your plan says no. If an NC-regulated plan, including the State Health Plan, denies a drug as not medically necessary, you can ask the NC Department of Insurance for a free external review within 120 days of the final appeal decision. Other denials, like plan exclusions, don’t qualify. It doesn’t cover self-funded employer plans, Medicare or Medicaid.

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

The FDA declared the semaglutide shortage over on Feb. 21, 2025, so copies of Wegovy can’t be made regularly unless your prescriber documents a significant difference for you. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information
In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
