Texas’s delivery rule requires pharmacies to pack prescriptions to hold the right temperature and to replace a drug compromised in delivery. It bars drone delivery of sterile compounded preparations. The rule was last amended effective Sept. 1, 2025.

Does insurance cover tirzepatide in Texas in 2026?
For weight loss alone, rarely. Texas Medicaid doesn’t cover weight-loss drugs for adults; it covers Zepbound only for obesity with sleep apnea. Texas’s state employee plan doesn’t cover GLP-1s for weight loss. With Medicare Part D, the GLP-1 Bridge can bring Zepbound KwikPens to $50 a month.
Texas Medicaid: not for weight loss
Not for weight loss. Texas Medicaid doesn’t cover weight-loss drugs for adults 21 and older in traditional Medicaid, apart from a few named exceptions. It covers Zepbound, with prior authorization, only for adults with obesity and moderate to severe obstructive sleep apnea confirmed by a sleep test. Those criteria were first published April 25, 2025, and last revised Oct. 24, 2025.

HealthSelect of Texas: not for weight loss
No. HealthSelect’s prescription plan excludes drugs prescribed only for obesity and names Zepbound as an example (plan document effective Sept. 1, 2025). Instead, it offers two online weight programs, and covers bariatric surgery for active employees after a separate $5,000 deductible.
TRS-ActiveCare and TRS-Care Standard (public school employees, and school retirees without Medicare): No. TRS says its plans don’t cover weight-loss drugs under any circumstance, even when weight loss might help another condition; it names Wegovy as an example. It covers these drugs for diabetes. TRS-ActiveCare’s 2026-27 plan year began Sept. 1, 2026.
UT SELECT (University of Texas System employees): No. For Sept. 1, 2026, to Aug. 31, 2027, UT SELECT’s drug plan excludes GLP-1s for any condition other than diabetes; some patients with diabetes may be covered through prior authorization. It also excludes compound medications.

Job-based and marketplace plans: it depends on your plan
If you’re insured through work, you probably have a self-funded plan, and those are exempt from state laws on required coverage. Ask HR or your plan: Are Zepbound and Wegovy covered for weight loss? Is prior authorization required? Which BMI or health conditions qualify?
If your plan says no. If your plan denies a drug, appeal to the plan first. Then ask for an external review by an outside reviewer: it’s free, and you have 4 months after the final denial. It covers denials based on medical judgment or experimental treatment. Texas’s insurance department helps only with fully insured plans, not self-funded ones.

Can you bring tirzepatide back from Mexico?
Not legally, in most cases. In most cases, it’s illegal to bring medicines from Mexico for personal use, the FDA says, and drugs approved there are unapproved in the U.S. The FDA is also aware of counterfeit Ozempic marketed in the U.S. Heat adds risk: McAllen’s airport hit 100°F or more on 97 days in summer 2026, and the Zepbound and Wegovy labels cap unrefrigerated storage at 86°F.

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 25 mg/mL, 10 mg is 10 ÷ 25 = 0.4 mL, or 40 units.

It covers Zepbound with prior authorization only for adults with obesity and moderate to severe obstructive sleep apnea confirmed by a sleep test, used with a reduced-calorie diet and more activity. Those criteria were last revised Oct. 24, 2025.
Do HealthSelect, TRS-ActiveCare or UT SELECT cover Zepbound?
Not for weight loss. HealthSelect’s drug plan for state employees excludes drugs prescribed only for obesity and names Zepbound (plan document effective Sept. 1, 2025). TRS says its plans don’t cover weight-loss drugs under any circumstance. UT SELECT’s drug plan for Sept. 1, 2026, to Aug. 31, 2027, excludes GLP-1s for anything other than diabetes.
Does Medicare cover Zepbound in Texas?
Through the Medicare GLP-1 Bridge, eligible Texans can get it for $50 a month from July 1, 2026, to Dec. 31, 2027, with Medicare’s approval. It covers the Zepbound KwikPen, Wegovy and Foundayo, but not Zepbound vials or single-dose pens. If you have diabetes, sleep apnea or fatty liver disease, ask your own Medicare drug plan.
Is compounded tirzepatide legal in Texas?
It can be, when it’s compounded for you. Federal rules require a prescription for an individual patient, and since the FDA ended the tirzepatide shortage on Dec. 19, 2024, copies of Zepbound can’t be made regularly unless your prescriber documents a significant difference for you. A federal appeals court upheld the FDA on Aug. 27, 2026. It isn’t FDA-approved.
Does my tirzepatide prescriber need a Texas license?
Yes. Under Texas law, a doctor in another state who treats a patient in Texas is practicing medicine in Texas, and that requires a Texas license. Texas joined the Interstate Medical Licensure Compact in 2021, a faster path to a Texas license for qualified doctors. You can check any doctor with the Texas Medical Board’s Look Up a License tool.
Can a pharmacy in another state ship tirzepatide to Texas?
Yes, if Texas has licensed it. An out-of-state pharmacy needs a Texas Class E license, and one that compounds sterile drugs like injections needs a Class E-S license, which requires an inspection by the Board or its designee. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information
In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
