How we compared. On Nov. 4, 2026, we read the public pricing pages of the 67 programs in our price index (no quiz, account or checkout) and checked PreventiveMD’s and Mondae’s in a live browser. For each one we record six things: the month-to-month price with every required fee added; whether it rises with your dose; how often it bills (every 4 weeks or 28 days is 13 charges a year, not 12); its prepaid rates and what it takes up front; the 12-month total both ways; and whether it sells in New York. When a program’s own pages disagree, its row says so.

Pharmacies
Compounding pharmacies licensed to mail compounded injections to New Yorkers
Only pharmacies registered with the New York State Education Department may mail you compounded tirzepatide. A pharmacy outside New York must register with the State Education Department as a nonresident establishment ($345 application fee) and stay in good standing at home. It must keep records of drugs shipped here, put a toll-free number on each label and name a New York agent. Outsourcing facilities must send inspection results yearly.

- Check it in a minute. Look up any pharmacy on New York’s pharmacy license lookup before you pay.
- Match your label. The pharmacy’s name and address on your box should match its New York registration.
- NexLife’s partner pharmacies. Absolute Pharmacy (Lutz, FL) and Boudreaux’s New Drug Store (Lake Charles, LA). We send New York prescriptions only to the ones with the licenses New York requires.

Can you get tirzepatide online in New York?
Yes. New York lets licensed providers treat you by telehealth, under the rules below. Your prescriber needs a New York license, and a pharmacy outside New York must register in New York to ship to you.
Getting care online
New York’s telehealth law covers diagnosis and treatment by licensed providers, but it doesn’t let anyone give care without the New York license it requires. Doctors and physician assistants must keep a record that accurately reflects your evaluation and treatment, for at least six years. Medicaid also requires documented consent to telehealth.

A New York license

Does insurance cover tirzepatide in New York in 2026?
For weight loss alone, rarely. NYRx, New York Medicaid’s pharmacy program, doesn’t cover any GLP-1 for weight loss. New York’s state employee plan covers GLP-1s for weight loss only with criteria on the Empire Plan. With Medicare Part D, the GLP-1 Bridge can bring Zepbound KwikPens to $50 a month.
NYRx: not for weight loss
No. NYRx, New York Medicaid’s pharmacy program, doesn’t cover any GLP-1 for weight loss and says weight loss has never been a covered use. Its July 30, 2026 overview allows Zepbound with prior authorization for obstructive sleep apnea, with a BMI of 40 or more and lifestyle changes, for up to two tries per lifetime.
New York State Department of Health, NYRx, 09/13/23, Rev. 07/16/26
NYSHIP: only with criteria
Yes, with prior authorization. The Empire Plan’s October 2026 drug list shows Zepbound as a preferred anti-obesity drug that needs prior authorization. The list doesn’t give the approval criteria, so ask the plan’s prescription program what your prescriber must send.
NYSHIP Empire Plan for participating employers (school districts, municipalities and other public employers that join NYSHIP): School districts and towns can buy into NYSHIP’s Empire Plan. The October 2026 drug list posted for these employers shows Zepbound and Wegovy as preferred anti-obesity drugs that need prior authorization.
NYSHIP for SUNY employees in UUP (SUNY faculty and staff represented by United University Professions (UUP)): UUP, a union for SUNY employees, is one of the groups NYSHIP covers. If you’re in the Empire Plan, its October 2026 drug list shows Zepbound and Wegovy needing prior authorization.

Medicare: $50 through the GLP-1 Bridge
Eligible Part D enrollees can get the Zepbound KwikPen, the Wegovy pen and pill, or Foundayo for $50 a month, with prior authorization, from July 1, 2026, to Dec. 31, 2027. Zepbound vials aren’t included. Who qualifies
TRICARE: Prime and Select still cover weight-loss medications in certain circumstances; TRICARE For Life has excluded them since Aug. 31, 2025.

Job-based and marketplace plans: it depends on your plan
A bill to make New York insurance plans cover FDA-approved obesity drugs (S3104/A4211) is still in committee. So coverage depends on your plan: ask whether Zepbound or Wegovy is covered for weight loss, what prior authorization needs, and how to appeal.
If your plan says no. Denied as not medically necessary by an insurer or HMO? Ask the Department of Financial Services for an external appeal within 4 months of the final internal decision. Plans may charge $25 (up to $75 a year), refunded if you win; Medicaid members pay nothing. It covers only benefits your plan would otherwise cover.

How do you check a tirzepatide provider in New York?
In about five minutes, for free. Ask for your prescriber’s name and license and the pharmacy that will fill your prescription, then look both up on New York’s public license searches. A good program answers in writing. Here are the questions to ask, with NexLife’s answers.
- Get two namesYour prescriber’s, and your pharmacy’s.

Can you bring tirzepatide back from Canada?
Not legally, in most cases. Buying in Canada? The FDA says drugs approved in another country are unapproved in the U.S., and importing them, for personal use or otherwise, violates federal law. Heat is a second risk: Zepbound’s and Wegovy’s labels don’t allow storage above 86°F. The legal route is a New York-licensed prescriber and a registered pharmacy.

What changed for tirzepatide in New York, and when?
Nationally, the FDA declared the tirzepatide shortage over in December 2024, an appeals court upheld that in August 2026, and Medicare’s $50 GLP-1 Bridge started in July 2026. Here’s every change we found for New York, newest first, each with its source.

How many units is your tirzepatide dose?
It depends on your vial’s strength. Here are the doses on Zepbound’s label at one example strength, with the math to check your own.
| Dose | mL at 25 mg/mL | Units (U-100) | On the label |
|---|---|---|---|
| 2.5 mg | 0.1 mL | 10 units | Starting dose, for 4 weeks |
| 5 mg | 0.2 mL | 20 units | A maintenance dose |
| 7.5 mg | 0.3 mL | 30 units | A step up |
| 10 mg | 0.4 mL | 40 units | A maintenance dose |
| 12.5 mg | 0.5 mL | 50 units | A step up |
| 15 mg | 0.6 mL | 60 units | The maximum, and a maintenance dose |
The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 25 mg/mL, 10 mg is 10 ÷ 25 = 0.4 mL, or 40 units.
25 mg per mL is an example only: it’s the strength of the 15 mg multi-dose vial on Zepbound’s label (15 mg per 0.6 mL). Read the strength on your own vial’s label; your provider sets your dose, in steps at least 4 weeks apart. Every step: the tirzepatide dosage chart.

A provider licensed in New York decides whether it’s right for you, and a licensed pharmacy mails it to you. Compounded versions aren’t FDA-approved.
Does NexLife have clinics near me?
No, and you don’t need one: NexLife’s care is online, wherever you are in New York. After about 3 minutes of health questions, a provider licensed in New York decides whether treatment is right for you; if you’re prescribed, a state-licensed compounding pharmacy sends it with free expedited shipping. Support is around the clock: call or text (949) 818-8000.
Does New York Medicaid cover Zepbound for weight loss?
No. NYRx, New York Medicaid’s pharmacy program, doesn’t cover Zepbound or other GLP-1s for weight loss, and it says weight loss has never been a covered use. Its July 30, 2026 overview does allow Zepbound with prior authorization for obstructive sleep apnea, with a BMI of 40 or more and lifestyle changes, for up to two tries per lifetime.
Does the Empire Plan cover Zepbound for state employees?
Yes, with prior authorization. The Empire Plan’s October 2026 drug list shows Zepbound as a preferred anti-obesity drug that needs prior authorization. The list doesn’t spell out the approval criteria, so ask the plan’s prescription program what your prescriber must send. The only 2027 drug change NYSHIP has announced concerns certain hyperinflated drugs.
Is compounded tirzepatide legal in New York?
Yes, with a prescription written for you and filled by a pharmacy New York has registered. The FDA ended the tirzepatide shortage on Dec. 19, 2024, and an appeals court upheld that on Aug. 27, 2026. A compounded version now counts as a copy of Zepbound unless your prescriber documents a significant difference for you. It isn’t FDA-approved.
Does my tirzepatide prescriber need a New York license?
Yes. New York requires full licensure and current registration of anyone who practices in the state, and its telehealth law doesn’t change that. New York hasn’t joined the Interstate Medical Licensure Compact; bills to join (S1505 and A1983) are still in committee. You can check any provider’s license free on the State Education Department’s online search.
Can a pharmacy in another state ship tirzepatide to New York?
Yes, if New York has registered it. An out-of-state pharmacy must register with the State Education Department as a nonresident establishment, stay in good standing in its home state, keep records of what it ships here and put a toll-free number on every label. Outsourcing facilities must also send inspection results every year.
How do I keep tirzepatide cold in a New York summer?
Get it into the fridge as soon as it arrives. Zepbound’s label allows up to 86°F outside the fridge for no more than 21 days; a compounded vial follows its own pharmacy label. In summer 2026, LaGuardia Airport hit 86°F or more on 40 days and peaked at 104°F. Buffalo did on 9 days.
How much does tirzepatide cost in New York without insurance?
Online, compounded tirzepatide runs $126 to $470 a month, all in, with a $267 median (price index, checked Nov. 4, 2026). Zepbound at LillyDirect is $299 to $449 a month; $449 from 7.5 mg needs a refill within 45 days, or it’s $499 to $699. NexLife charges $115 a month on the 12-month plan or $129 month to month.

Important safety information
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product.
Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information
In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
