NexLife publishes this page. Storage and shipping guidance comes from the FDA page linked in Sources, checked October 6, 2026. This is general information, not medical advice.

Quick answer: how should compounded semaglutide be stored?

Store it exactly as the label from your dispensing pharmacy says. Compounded semaglutide is prepared by a state-licensed pharmacy, and how it should be stored, and for how long, depends on how that pharmacy made it, so rules written for Wegovy don’t carry over. The FDA says injectable GLP-1 drugs require refrigeration as their labels indicate, recommends not using any that arrives warm or with too few cold packs, and recommends discarding a multiple-dose vial within 28 days of first use, even if medication remains.

  • Your pharmacy’s label sets the rules
  • Arrived warm? Don’t use it
  • Discard an open vial after 28 days
  • A new needle and syringe every dose

Where your storage rules come from

Your label, from the pharmacy that made your vial.

  1. The pharmacy’s label.

It gives the storage directions and the beyond-use date for your vial. Follow them exactly.

  2. Not Wegovy’s rules.

Wegovy’s storage limits were set for its own pens and vials. A compounded vial is a different product, so those limits don’t carry over.

  3. The beyond-use date.

The date after which the pharmacy says not to use it. It depends on how the pharmacy made the medication.

  4. Not sure? Ask first.

Call the pharmacy on your label, or call or text us 24/7 at (949) 818-8000.

If your semaglutide package arrives warm

Don’t use it. Call the pharmacy, and let us know.

  1. Don’t use it.

The FDA recommends not using any injectable GLP-1 that arrives warm or without enough cold packs, because it can affect the medication’s quality.

  2. Call the pharmacy, then us.

Call the pharmacy on your label, and call or text NexLife 24/7 at (949) 818-8000. Have your order details and a photo of the package ready.

  3. Wait for their answer.

Keep it unused while you wait. The pharmacy and our care team will tell you what happens next.

  4. Keep the packaging.

Keep the box, cold packs and vial until you hear back.

Left out of the fridge?

Don’t guess. Whether compounded semaglutide can stay out of the fridge, and for how long, depends on how the pharmacy made it. Keep the vial, and call the pharmacy on your label before your next dose. Don’t go by Wegovy’s limits, or by what another pharmacy said about a different vial.

Stored right, every time.

Your medication travels from a named pharmacy to your door. Here’s how to keep it right once it’s there.

  • Labels differ. Each pharmacy sets its own storage directions.
  • Deliveries vary. Weather and timing can affect a package.
  • Advice gets mixed. Rules for brand-name pens don’t apply to compounded vials.

After you open a vial

Follow your label, plus habits the FDA recommends for multiple-dose vials.

  1. Store it the way your label says.

Right after each dose, put it back where your label says to keep it.

  2. Mind the 28 days.

Use a multiple-dose vial within 28 days of first use, or by the beyond-use date on your label, whichever comes first. The FDA recommends discarding it after 28 days, even if medication remains.

  3. Write down the date.

Note the day you first use a vial, so you know when the 28 days are up.

  4. A new needle, every time.

Use a new, sterile needle and syringe for every dose. Reusing them can contaminate the medication.

That it isn’t warm, that the vial and seal aren’t damaged, and that the label matches your prescription.

  4. Store it right away.

    Exactly as your label says.

Report a problem

Tell the pharmacy, tell us, and tell the FDA.

Semaglutide storage: your questions, answered

How should I store compounded semaglutide?

Store it exactly as the label from your dispensing pharmacy says. Compounded semaglutide is prepared by a state-licensed pharmacy, and how it should be stored, and for how long, depends on how that pharmacy made it, so rules written for Wegovy don’t carry over. The FDA says injectable GLP-1 drugs require refrigeration as their labels indicate, recommends not using any that arrives warm or with too few cold packs, and recommends discarding a multiple-dose vial within 28 days of first use, even if medication remains.

Does compounded semaglutide need to be refrigerated?

Follow your label. The FDA says injectable GLP-1 drugs require refrigeration as their package inserts indicate, and for compounded semaglutide, the directions that count are the label and instructions from your dispensing pharmacy.

What if my semaglutide arrives warm?

Don’t use it. The FDA recommends not using any injectable GLP-1 that arrives warm or without enough cold packs. Call the pharmacy on your label, and call or text NexLife 24/7 at (949) 818-8000.

Can I use semaglutide that was left out of the fridge?

Not until the pharmacy that made it says so. Storage limits for compounded semaglutide depend on how the pharmacy made it, so follow its label and directions, not limits written for other products.

How long can compounded semaglutide be unrefrigerated?

Only as long as your pharmacy’s label or directions allow, if at all. It depends on how the pharmacy made it, and Wegovy’s limits don’t apply to compounded vials. If you’re unsure, call the pharmacy before you use it.

What is the shipping temperature for compounded semaglutide?

Your pharmacy decides how to pack and ship it. If it arrives warm or with too few cold packs, the FDA recommends not using it: call the pharmacy, and call or text us 24/7.

How long can I use a vial after opening it?

Up to 28 days after first use, or until the beyond-use date on your label, whichever comes first. The FDA recommends discarding a multiple-dose vial after 28 days, even if medication remains.

What is a beyond-use date?

The date the pharmacy sets after which the medication shouldn’t be used. It depends on how the pharmacy made it. Use your vial by that date, or within 28 days of first use, whichever comes first.

Can I freeze compounded semaglutide?

Follow your label, and don’t freeze it unless your pharmacy says you can. If it froze by accident, call the pharmacy before you use it.

Can I reuse a needle?

No. The FDA says to use a new, sterile needle and syringe for every dose, because reusing them can contaminate the medication.

What if my package is damaged?

Don’t use what’s inside if the vial, seal or label is damaged. Keep the packaging, call the pharmacy on your label, and call or text us at (949) 818-8000.

How do I report a problem with my medication?

Tell the pharmacy on your label and NexLife, and report side effects or product problems to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.

What does compounded semaglutide cost at NexLife?

$139 a month at every dose, or $119 a month on a 12-month plan, with no membership fee. Shipping is free and expedited.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved. The FDA has not reviewed them for safety, effectiveness or quality. The risks below come from the labeling of the FDA-approved medicines.

Both medicines may cause tumors in the thyroid, including thyroid cancer. In studies with rodents they caused thyroid tumors, and it isn't known whether they cause them in people. Don't use either if you or anyone in your family has had medullary thyroid carcinoma (MTC), or if you have multiple endocrine neoplasia syndrome type 2 (MEN 2). Tell your clinician if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.

Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and depression or thoughts of suicide. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don't combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: use another method for 4 weeks after you start and after each dose increase. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your clinician if you're pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your clinician. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
