How we chose these semaglutide providers
On Nov. 4, 2026, we checked 84 programs’ public pages, with no quiz, account or checkout: 67 publish prices we could read, and 40 sell compounded semaglutide in California. We picked one for each need, judged on what each program publishes: the all-in price at every dose, what’s included, and how it bills and renews. NexLife publishes this guide and is our top pick.
Prices are each program’s published national prices; a few limit what they sell in some states, so confirm before you pay. Full index with sources: GLP-1 price index. Nationally, side by side: best semaglutide providers. By dose: semaglutide price by dose. Month by month: compounded semaglutide price.
Key takeaways
- Since Jan. 1, 2026, Medi-Cal hasn’t covered Wegovy for weight loss (checked Nov. 2, 2026).
- California’s state employee plan covers GLP-1 drugs for weight loss only with criteria in some HMO plans (BMI 40 or more).
- A pharmacy outside California needs a California license to ship you a prescription; you can look it up in minutes.
- Your prescriber needs a California license to treat you there.
- Heat is the local risk: Palm Springs reached 86°F or more on all 121 summer days with data in 2026; San Francisco (SFO) on 4.
- At NexLife, it’s $95 a month on the 12-month plan or $109 month to month, at every dose: $90 under the $199 median of the 24 programs in our GLP-1 price index that publish a month-to-month price in California.

How we compared. On Nov. 4, 2026, we read the public pricing pages of the 67 programs in our price index (no quiz, account or checkout) and checked PreventiveMD’s and Mondae’s in a live browser. For each one we record six things: the month-to-month price with every required fee added; whether it rises with your dose; how often it bills (every 4 weeks or 28 days is 13 charges a year, not 12); its prepaid rates and what it takes up front; the 12-month total both ways; and whether it sells in California. When a program’s own pages disagree, its row says so.

Pharmacies
Compounding pharmacies licensed to mail compounded injections to Californians
Only pharmacies licensed by the California State Board of Pharmacy may mail you compounded semaglutide. A pharmacy outside California needs a California nonresident pharmacy license. To ship compounded injections, it also needs a nonresident sterile compounding license, renewed yearly and never issued or renewed before a Board inspection. Since July 1, 2026, it can’t ship to Californians unless its pharmacist in charge holds a California license.

What changes depending on where you live in California?
Not your price or how you’re seen: care is online, and NexLife’s price is the same anywhere in California. What changes is the heat at your door. In summer 2026, Palm Springs reached 86°F or more, the most Wegovy’s label allows outside the fridge, on all 121 days with data, San Jose on 21 and San Francisco (SFO) on 4. Local health numbers differ too: CDC estimates adult obesity at 17.2% in San Francisco County and 37.7% in Fresno County, against 27.8% statewide.

NexLife publishes this guide. A licensed provider decides whether a prescription is right for you; finishing the health questions doesn’t mean you’ll be prescribed. Compounded semaglutide isn’t FDA-approved, and it’s prescribed only when it offers a significant difference for you compared with an FDA-approved product.
How it works
How to get semaglutide in California, from your couch to your door
Online, in four steps, with no insurance needed. Here’s what happens at NexLife.
- Answer a few health questionsAbout 3 minutes online: your health history, your medicines and your goals.
- A licensed provider reviews themA provider licensed to treat you in California decides whether compounded semaglutide is right for you, following California’s rules. You’re prescribed only if it is.
- A licensed pharmacy prepares itYour prescription goes to a partner pharmacy with the licenses California requires, which usually ships within 1 to 2 business days, free.
- It comes to your doorIn an insulated, cold-packed box. Put it in the fridge right away; if it arrives warm, don’t use it: call or text (949) 818-8000.
How do you keep semaglutide cool in a California summer?
Store compounded semaglutide the way your prescription label says, usually in the fridge at 36°F to 46°F, and get the box inside fast. Wegovy’s rule for pens kept at 46°F to 86°F for up to 28 days doesn’t apply to compounded vials. Orders through NexLife ship in insulated, cold-packed boxes; if yours arrives warm, don’t use it: call or text (949) 818-8000.
On hot days, be home for delivery or pick a shaded drop-off spot, and get the box into the fridge right away. In a disaster, california’s emergency refill law lets a pharmacist refill a prescription when going without it might interrupt your ongoing care and significantly harm your well-being. Since AB 1503, the pharmacist no longer has to try to reach your prescriber first in that case. (California State Board of Pharmacy) More: how to store compounded semaglutide.
Can you get semaglutide online in California?
Yes. California lets licensed providers treat you by telehealth, under the rules below. Your prescriber needs a California license, and a pharmacy outside California needs a California license to ship to you.
Getting care online
Your provider must hold a California license. A prescription needs an appropriate prior exam and a medical reason; under state law, that exam can happen through telehealth, as long as it meets the standard of care. Before using telehealth, the provider must tell you, get your verbal or written consent and record it.
Cal. Bus. & Prof. Code §2242(a) (prior examination); §2290.5(b) (consent); §2052 (license) · source
A California license
Your prescriber needs a California license: treating patients in California without one is a crime. A narrow exception, added in 2023, lets out-of-state doctors treat an immediately life-threatening condition by telehealth. Check any license on the state’s DCA License Search.

Nurse practitioners and PAs
Nurse practitioners prescribe under standardized procedures with physician supervision, unless certified to work without them after a transition to practice: first alongside physicians, later on their own. PAs prescribe under a written practice agreement with a supervising physician.

Inspections
From July 1, 2026, a pharmacy outside California, compounding pharmacies included, may not ship prescriptions to California patients unless its pharmacist in charge holds a California license. The Board may now inspect these pharmacies.

Does insurance cover semaglutide in California in 2026?
For weight loss alone, rarely. Since Jan. 1, 2026, Medi-Cal hasn’t covered Wegovy for weight loss. California’s state employee plan covers GLP-1s for weight loss only with criteria in some HMO plans (BMI 40 or more). With Medicare Part D, the GLP-1 Bridge can bring the Wegovy pen or pill to $50 a month.
Medi-Cal: not for weight loss
Not for weight loss in adults. Since Jan. 1, 2026, Medi-Cal hasn’t covered Wegovy for weight loss; for members under 21, it reviews weight-loss requests for medical necessity. It also reviews prior authorization requests for cardiovascular disease based on medical necessity, and since April 1, 2026, Wegovy for the liver disease MASH, without cirrhosis, needs no prior authorization. Ozempic stays limited to type 2 diabetes.
California Department of Health Care Services
CalPERS health plans: only with criteria
For weight loss, HMO plans that run their own drug benefit, like Blue Shield and Kaiser, cover GLP-1s at a BMI of 40 or more, with prior authorization. All CalPERS plans cover GLP-1s for diabetes and some related conditions, including heart risk factors and MASH.
California Public Employees’ Retirement System
Medicare: $50 through the GLP-1 Bridge
Eligible Part D enrollees can get the Wegovy pen or pill, the Zepbound KwikPen or Foundayo for $50 a month, with prior authorization, from July 1, 2026, to Dec. 31, 2027. Who qualifies
TRICARE: Prime and Select still cover weight-loss medications in certain circumstances; TRICARE For Life has excluded them since Aug. 31, 2025.
CMS, Medicare GLP-1 Bridge; TRICARE, Aug. 5, 2025
Job-based and marketplace plans: it depends on your plan
It depends on your plan. Ask whether Zepbound and Wegovy are covered for weight loss, whether prior authorization is needed, and which BMI or health conditions qualify. If your job plan is self-insured, California’s Department of Managed Health Care doesn’t regulate it.
If your plan says no. Appeal to your plan first; it must decide within 30 days, or 3 days if your health is in serious danger. Then ask the state for a medical review (IMR): it’s free, you have six months, and the plan must follow the decision. IMR can’t overturn a benefit exclusion or cover self-insured job plans. For a Medi-Cal Rx denial, ask for a State Hearing, generally within 90 days of the notice.
Independent Medical Review: Department of Managed Health Care (HMOs and some PPO/EPO plans; form DMHC 20-224, rev. 09/25) and California Department of Insurance (insurance policies). Medi-Cal Rx denials: State Hearing within 90 days.

How do you check a semaglutide provider in California?
In about five minutes, for free. Ask for your prescriber’s name and license and the pharmacy that will fill your prescription, then look both up on California’s public license searches. A good program answers in writing. Here are the questions to ask, with NexLife’s answers.
- Get two namesYour prescriber’s, and your pharmacy’s.

Can you bring semaglutide back from Mexico?
Not legally, in most cases. Bringing tirzepatide or semaglutide back from Tijuana isn’t legal in most cases. The FDA says drugs approved in another country are unapproved in the U.S., and importing unapproved drugs, for personal use or otherwise, violates federal law. Heat matters too: San Diego’s airport hit 95°F in summer 2026, and Zepbound’s and Wegovy’s labels never allow storage above 86°F.

What changed for semaglutide in California, and when?
Nationally, the FDA declared the semaglutide shortage over in February 2025, an appeals court upheld that in August 2026, and Medicare’s $50 GLP-1 Bridge started in July 2026. Here’s every change we found for California, newest first, each with its source.

How many units is your semaglutide dose?
It depends on your vial’s strength. Here are the Wegovy label’s dose steps at two example strengths, with the math to check your own.
| Weeks | Weekly dose | At 2.5 mg/mL | At 5 mg/mL |
|---|---|---|---|
| 1 to 4 | 0.25 mg | 0.1 mL (10 units) | 0.05 mL (5 units) |
| 5 to 8 | 0.5 mg | 0.2 mL (20 units) | 0.1 mL (10 units) |
| 9 to 12 | 1 mg | 0.4 mL (40 units) | 0.2 mL (20 units) |
| 13 to 16 | 1.7 mg | 0.68 mL (68 units) | 0.34 mL (34 units) |
| 17 onward | 2.4 mg | 0.96 mL (96 units) | 0.48 mL (48 units) |
The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.
Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

With Medicare Part D: the Medicare GLP-1 Bridge, $50 a month for eligible enrollees, with prior authorization. Without automatic renewals: Rx.com. For help getting insurance to pay: Ro.
Where can I get semaglutide near me in California?
Wherever you live in California, you can start online: a provider licensed in California evaluates you and, if it’s right for you, a licensed pharmacy ships semaglutide to your door. In person, health-system weight programs and local clinics prescribe it. With a prescription for brand-name Wegovy, NovoCare Pharmacy delivers it, or a retail pharmacy can fill it.
Can I get GLP-1 weight-loss injections online in California?
Yes. 40 programs in our price index sell compounded semaglutide online in California; our top pick, NexLife, is $95 a month on the 12-month plan or $109 month to month. A provider licensed in California decides whether it’s right for you, and a licensed pharmacy mails it to you. Compounded versions aren’t FDA-approved.
Does NexLife have clinics near me?
No, and you don’t need one: NexLife’s care is online, wherever you are in California. After about 3 minutes of health questions, a provider licensed in California decides whether treatment is right for you; if you’re prescribed, a state-licensed compounding pharmacy sends it with free expedited shipping. Support is around the clock: call or text (949) 818-8000.
Does Medi-Cal cover Wegovy for weight loss?
No, not for adults. Since Jan. 1, 2026, Medi-Cal hasn’t covered Wegovy for weight loss, though it reviews weight-loss requests for members under 21 for medical necessity. It also reviews prior authorization requests for cardiovascular disease. Since April 1, 2026, Wegovy for the liver disease MASH, without cirrhosis, needs no prior authorization when the claim carries that diagnosis code.
Does Medi-Cal still cover Ozempic?
Yes, for type 2 diabetes. Medi-Cal keeps a type 2 diabetes diagnosis restriction on Ozempic and Mounjaro, so claims without that diagnosis are denied. Since Jan. 1, 2026, Ozempic, Rybelsus and similar drugs aren’t covered for weight loss. For members under 21, Medi-Cal reviews weight-loss requests for medical necessity.
Does CalPERS cover Wegovy?
Possibly, in some plans, at a BMI of 40 or more. CalPERS says HMO plans that run their own drug benefit, including Blue Shield and Kaiser, cover GLP-1 drugs for obesity at that level, with prior authorization, but it doesn’t name Wegovy. All CalPERS plans cover GLP-1s for diabetes and certain obesity-related conditions, including heart risk factors and MASH.
Is compounded semaglutide legal in California?
It can be, when it’s compounded for you. The FDA declared the semaglutide shortage over on Feb. 21, 2025, so copies of Wegovy can’t be made regularly unless your prescriber documents a significant difference for you. A pharmacy outside California needs California licenses to ship it here. Compounded semaglutide isn’t FDA-approved.
Can a nurse practitioner prescribe semaglutide in California?
Yes. Nurse practitioners prescribe under standardized procedures with physician supervision, using a furnishing number from the Board of Registered Nursing, unless they’re certified to work without them after a transition to practice. Physician assistants prescribe under a written practice agreement with a supervising physician.
Can a pharmacy in another state ship semaglutide to California?
Yes, if it holds California licenses. It needs a nonresident pharmacy license, and to ship compounded injections, a nonresident sterile compounding license that the Board won’t issue or renew before inspecting the site. Since July 1, 2026, it must also have a pharmacist in charge licensed in California. You can check any pharmacy online.
How do I keep semaglutide safe in California heat?
Refrigerate it as soon as it arrives. Wegovy single-dose pens can stay out of the fridge for up to 28 days, but only between 46°F and 86°F. In summer 2026, the Bakersfield airport hit 86°F or more on 113 of 122 days, and Riverside’s on 104. Compounded vials follow their own pharmacy label.
How much does semaglutide cost in California without insurance?
Online, compounded semaglutide costs $87 to $348 a month, median $199 (checked Nov. 4, 2026). The Wegovy pen at NovoCare Pharmacy is $199 for each of a new patient’s first two fills (through Dec. 31, 2026), then $349 a month ($399 at 7.2 mg); Wegovy pills cost $149 to $299. NexLife charges $95 a month on the 12-month plan or $109 month to month.
Where can I get semaglutide in San Jose or Sacramento?
You can get it without leaving home. A provider licensed in California can treat you online, and a pharmacy the California State Board of Pharmacy has licensed can ship it to San Jose, Sacramento or anywhere else in the state. Before you visit a clinic or med spa, look up the prescriber on DCA License Search.

Important safety information
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product.
Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information
In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.

How we checked. On Nov. 4, 2026, we read the public pages of every program in our price index (no quiz, account or checkout) and checked PreventiveMD’s and Mondae’s in a live browser; on Nov. 2, 2026, we read California’s Medicaid, licensing, telehealth and pharmacy rules at their sources. The heat counts are ours, from NOAA’s daily station data at 12 weather stations.
How we make money. We earn revenue when people join NexLife, one of the programs here, so we show why NexLife is our top pick and name the catch for every other option.
