GLP-1 medications of this class caused thyroid C-cell tumors in rodents. It is not known whether they cause these tumors in humans. Do not use compounded semaglutide or compounded tirzepatide if you or a family member has had medullary thyroid carcinoma (MTC), or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Do not use if you have had a serious allergic reaction to semaglutide or tirzepatide. Tell your provider right away about a lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath.
Serious risks include inflammation of the pancreas (pancreatitis), gallbladder disease and gallstones, kidney injury from persistent vomiting or diarrhea, low blood sugar (particularly with insulin or a sulfonylurea), slowed stomach emptying, worsening of diabetic retinopathy, and sudden vision loss from non-arteritic anterior ischemic optic neuropathy (NAION). Contact your provider immediately for severe abdominal pain, persistent vomiting, or any change in vision. Tell your provider about any new or worsening changes in mood, behavior, or thoughts.
Before any surgery or procedure involving anesthesia or deep sedation — including endoscopy, colonoscopy, and dental sedation — tell your surgeon and anesthesiologist that you take a GLP-1 medication. These medications slow stomach emptying, and food may remain in the stomach after standard fasting, increasing the risk of aspiration.
Tell your provider if you are pregnant, planning to become pregnant, or breastfeeding. These medications should not be used during pregnancy. Also tell your provider about all medications, vitamins, and supplements you take, including insulin and sulfonylureas. If you use oral hormonal birth control, tirzepatide may reduce its effectiveness; ask your provider about switching to a non-oral method or adding a barrier method for 4 weeks after starting and after each dose increase.
Dosing. Compounded GLP-1 medications are typically dispensed as subcutaneous injections in multi-dose vials, with dosing, titration, concentration, and formulation determined by the prescribing provider.Follow only the dosing instructions provided for your specific prescription.
