Dosing, made clear.

Compounded semaglutide dosage chart: mg to units, checked

On a U-100 insulin syringe, units = dose in mg ÷ vial strength in mg/mL × 100. At 2.5 mg/mL, 0.25 mg is 10 units and 2.4 mg is 96 units. Your prescription sets your dose. If anything doesn’t match, call or text NexLife before you inject.

Every cell uses one formula: units = dose in mg ÷ vial strength in mg/mL × 100.

Small doses, big differences. 10 units is 0.25 mg from a 2.5 mg/mL vial, but 0.5 mg from a 5 mg/mL vial. Recheck the strength on every new vial’s label.

The schedule

How semaglutide doses usually step up

This is the FDA-approved Wegovy injection schedule; your prescription comes first.

WeeksWeekly doseWhat happens
1 to 40.25 mgStarting dose, to let your body adjust
5 to 80.5 mgStep two

Your provider may slow the steps or keep you at a lower dose if side effects are hard. Compounded semaglutide follows your prescription.

Vial math

Why you may have medicine left on day 28

  • The 28-day limit

The FDA recommends using an opened multiple-dose vial within 28 days of the first needle (or by the label’s date, if sooner), even if some is left.

  • An example

A 2 mL vial at 2.5 mg/mL holds 5 mg. At 0.25 mg a week, four doses use 1 mg, and the rest is discarded on day 28.

  • Plan the next vial

Tell us when your dose changes. We line up your next vial with the pharmacy so the strength fits.

Avoid these

The mistakes behind most dosing errors

  • Confusing mL and units. 0.1 mL is 10 units on a U-100 syringe, not 1 unit.
  • Skipping the strength check. A new vial with a different strength changes your units.
  • Taking a salt form. If the label says “semaglutide sodium” or “acetate,” don’t use it. Call or text us.
  • Stepping up early. The FDA has reports of side effects from dose increases faster than the label schedule.

Unsure about a dose? Ask first.

Call or text NexLife at (949) 818-8000, 24/7. We check with your provider and the pharmacy, and get back to you before your next dose.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.

Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
