Yes, when it’s prescribed to treat a disease a physician has diagnosed, such as obesity. The IRS counts prescribed medicines as medical expenses, so HSA and FSA money can pay for compounded semaglutide in that case; weight loss for appearance or general health doesn’t count. With an HSA, the records are yours to keep. With an FSA, your plan confirms each expense, usually with a receipt, and some plans ask for a letter of medical necessity. NexLife accepts HSA and FSA cards at checkout, at $139 a month at every dose.

From IRS Publication 502 and the IRS’s questions and answers on nutrition and wellness.
| Expense | Counts? | What the IRS says |
|---|---|---|
| Prescribed medicine | Can count | The IRS includes medicines and drugs prescribed by a physician. Medical care has to be mainly for an illness, not for general health. |
| Weight-loss treatment for a diagnosed disease | Can count | Amounts you pay to lose weight count when the weight loss treats a specific disease a physician has diagnosed, such as obesity, hypertension or heart disease. |
| Nutrition counseling | Sometimes | It counts only when it treats a specific disease a physician has diagnosed, such as obesity. |
| Weight loss for appearance or general health | Doesn’t count | The IRS excludes weight-loss treatment for appearance, general health or a sense of well-being. |
| Diet food and drinks | Doesn’t count | They replace what you’d normally eat, so the IRS doesn’t count them. |
HSA and FSA plans pay for qualified medical expenses, which follow these IRS rules. An FSA pays only the expenses its plan covers, which can be fewer.
The same IRS rules, two different ways of checking.
| At a glance | HSA | FSA |
|---|---|---|
| How you pay | Your HSA card, or pay another way and reimburse yourself later | Your FSA card, or pay and send your plan a claim |
| Who checks the expense | You do: keep records showing each withdrawal paid a qualified medical expense | Your plan, with a third-party statement such as a receipt |
| If it doesn’t qualify | The withdrawal is taxed and may face an additional 20% tax | Your plan won’t reimburse it |
| Paying for months ahead | Keep a receipt for each month you pay for | An FSA can’t reimburse future months in advance; each month may be reimbursed once it’s provided |
| 2026 limit | $4,400 self-only or $8,750 family, with a qualifying high-deductible health plan | $3,400 from your pay; up to $680 can carry over if your plan allows |
Depending on how your FSA card payment is checked, your plan may ask for a receipt afterward.

A short note that ties the expense to a diagnosed disease.
A short letter from a physician who treats you. It names the condition, the treatment and why the treatment is needed.
Some FSA plans ask for one when an expense could be medical care or general wellness, as weight-loss treatment can be. Ask your plan what it needs and how long a letter stays valid.
The records are yours to keep. A letter can help show the treatment was for a disease a physician diagnosed.
Ask our care team for itemized receipts and a letter of medical necessity for your plan before you buy. Call or text (949) 818-8000.
Five steps, in order.
With an FSA, ask whether it covers prescribed weight-loss medication and what proof it needs. With an HSA, the IRS rules above apply.

Yes, when it’s prescribed to treat a disease a physician has diagnosed, such as obesity. HSA money can pay for qualified medical expenses, which include medicines prescribed by a physician. Keep your records: you must be able to show that each withdrawal paid for a qualified medical expense. A withdrawal that doesn’t qualify is taxed and may face an additional 20% tax.
Yes, if it’s prescribed to treat a diagnosed disease and your FSA plan covers it. An FSA confirms each expense with a third-party statement, such as a receipt, showing the expense and the amount, and some plans also ask for a letter of medical necessity. NexLife accepts FSA and HSA cards at checkout.
It can be. The IRS rules don’t address compounded drugs directly: they count medicines prescribed by a physician and exclude illegal treatments. Compounded semaglutide needs a prescription, so it can count when it’s lawfully compounded for you and treats a disease a physician has diagnosed. With an FSA, your plan’s rules apply too.
It depends on your plan. Some FSA plans ask for one for weight-loss treatment, because the IRS counts it only when it treats a disease a physician has diagnosed. With an HSA, a letter can support your records. Ask our care team for itemized receipts and a letter of medical necessity for your plan.
It depends on what the program includes and why. The IRS counts weight-loss program fees and nutrition counseling only when they treat a disease a physician has diagnosed, and it doesn’t count exercise for general health. FSA and HSA cards may be used for eligible portions of a NexLife plan, so check with your FSA plan, or keep records for your HSA.
Every receipt and your prescription details. For an HSA, the IRS says you must keep records showing that each withdrawal paid for a qualified medical expense that wasn’t paid or reimbursed another way. For an FSA, your plan needs a third-party statement, such as a receipt, showing the expense and the amount.
No, not under IRS rules. Weight-loss treatment counts only when it treats a specific disease a physician has diagnosed, such as obesity, hypertension or heart disease. The IRS excludes weight loss for appearance, general health or a sense of well-being.
Not in advance. An FSA can’t reimburse future expenses, so if you pay for more than one month at once, your plan may reimburse each month only once it’s provided. Ask your plan how it handles longer plans before you choose one.
Up to $4,400 in an HSA with self-only coverage, or $8,750 with family coverage, if you have a qualifying high-deductible health plan. A health FSA allows up to $3,400 a year from your pay, and plans that allow a carryover let you carry up to $680 into the next year.
$139 a month at every dose, with no membership fee, or $132, $125 and $119 a month on 3-, 6- and 12-month plans. Any authorization charge is refunded in full if you’re not prescribed. Klarna, Afterpay, FSA and HSA cards are accepted.
Yes. NexLife accepts Klarna and Afterpay at checkout; each decides approval and shows its installment schedule before you confirm. See paying with Klarna or Afterpay.
Any authorization charge at checkout is refunded in full if you’re not prescribed or don’t go ahead.
Compounded tirzepatide and semaglutide are not FDA-approved. The FDA has not reviewed them for safety, effectiveness or quality. The risks below come from the labeling of the FDA-approved medicines.
Both medicines may cause tumors in the thyroid, including thyroid cancer. In studies with rodents they caused thyroid tumors, and it isn't known whether they cause them in people. Don't use either if you or anyone in your family has had medullary thyroid carcinoma (MTC), or if you have multiple endocrine neoplasia syndrome type 2 (MEN 2). Tell your clinician if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.
Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and depression or thoughts of suicide. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don't combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: use another method for 4 weeks after you start and after each dose increase. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your clinician if you're pregnant, planning to be, or breastfeeding.
This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your clinician. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
