Cost

What does Willow cost?

It depends on the medicine, the form and your dose. Here’s Willow’s own price list, with what a year at each price adds up to.

From Willow’s price list (dated September 16, 2026) and its product pages, read November 1, 2026. “12 months” is 12 × the monthly price: the pages we read list monthly prices only. The medicines in this table are compounded, not FDA-approved.

From Willow’s price list (dated September 16, 2026) and its product pages, read November 1, 2026. “12 months” is 12 × the monthly price: the pages we read list monthly prices only. The medicines in this table are compounded, not FDA-approved.

The $299 is for semaglutide. Willow’s home page says plans start at $299 a month. Its price list also says plans start at $199, but the only $199 item on it is an NAD+ injection, which isn’t a GLP-1 medicine.

Your dose can change your price. Tirzepatide shots are $399 a month at the three lowest doses Willow lists, and $549 at the highest. Its terms say your fee may change with your dose, and that Willow tells you the new amount before it charges it.

No longer plans. The pages we read list monthly prices only. So a year of semaglutide shots is $3,588, and a year of tirzepatide shots $4,788 to $6,588.

Against the market, Willow’s lowest prices sit above the medians in our GLP-1 price index (checked November 1, 2026). For tirzepatide, it’s $399 against a $237 median across 14 programs. For semaglutide, it’s $299 against $199, across 17. Willow isn’t one of the index’s programs.

What’s included

What does Willow’s price include, and what costs extra?

The price covers the medication and the care around it. A few things can change what you pay.

Included in the monthly price, Willow says

  • Your compounded medication, prepared by a state-licensed compounding pharmacy
  • Your medical evaluation and treatment plan
  • Unlimited messaging with your Willow doctor, and free follow-ups
  • 24/7 online support from its Care Team
  • Free two-day shipping, in discreet packaging

What can cost more

  • A higher dose. Tirzepatide shots rise from $399 to $549 a month, semaglutide shots from $299 to $399 at 7 mg, and tirzepatide tablets from $399 to $459.
  • Your own pharmacy. Willow charges a $150 monthly platform fee if your prescription is filled elsewhere, and you pay that pharmacy for the medication.
  • NAD+. A separate injection at $199 a month. It isn’t a GLP-1.
  • Insurance. Willow doesn’t bill it. You can pay with HSA or FSA funds and send the receipts to your insurer yourself.

From Willow’s FAQ, price list and pharmacy policy, read November 1, 2026.

At NexLife, one price covers your medication, a licensed provider who follows your dose, a nutrition plan, a 1:1 coaching call, free expedited shipping and real people 24/7. There’s no membership fee, and the price is the same at every dose.

Tablets

Tablets or shots: what Willow sells, and what the FDA says

Willow sells both. Its tablets dissolve under the tongue once a day, and its shots are weekly. None of them is FDA-approved.

What Willow sells. Semaglutide and tirzepatide tablets, taken once a day under the tongue, and a combination tablet with both. It also sells weekly semaglutide and tirzepatide shots. A Willow doctor helps you choose.

In Willow’s own words: “There is no FDA-approved oral-dissolving formulation of semaglutide or tirzepatide.” That line is on its price list and its tablet guide (read November 1, 2026).

Every GLP-1 that Willow sells is compounded, and compounded medicines aren’t FDA-approved. The FDA explains what that means:

“This means that FDA does not verify the safety, effectiveness or quality of compounded drugs before they are marketed.”

U.S. Food and Drug Administration, Compounding and the FDA: Questions and Answers

FDA-approved GLP-1 pills do exist. The Wegovy pill (semaglutide) and Foundayo (orforglipron) are approved for weight. Their FDA labels say to swallow the tablets whole (Wegovy, Foundayo). Every pill and shot, side by side: GLP-1 pills vs. injections.

At NexLife, compounded sublingual tirzepatide tablets are $199 a month on the 12-month plan or $229 month to month. Semaglutide tablets are $165 a month on the 12-month plan or $199 month to month, at every dose. Shots cost less: $115 and $95 a month on the 12-month plan. More on sublingual semaglutide tablets and sublingual tirzepatide.

Reviews

What do Willow reviews say?

On Trustpilot, Willow is rated 4.1 out of 5, from 382 reviews (read November 1, 2026). Its newest reviews are mostly five-star, while 30% of all its reviews give one star.

  • 4.1 out of 5 on Trustpilot from 382 reviews
  • 60% of its reviews give five stars and 30% give one star
  • 53 of 60 newest reviews give five stars posted June 30 to October 29, 2026
  • 100% of negative reviews get a reply typically within 24 hours

What reviewers praise. Fast replies from doctors and the care team, on-time deliveries and an easy start. Some five-star reviewers add that Willow costs more than other sites they looked at.

What low ratings describe. Of the 60 newest reviews, 6 gave one or two stars, so most of the one-star reviews are older. The ones we read, from 2025 to October 2026, most often say the medication, often the tablets, didn’t seem to work for them. Others describe charges they disputed after canceling or pausing, shipping or order problems, and support that was hard to reach.

From Willow’s Trustpilot profile, read November 1, 2026: its rating, its star split, its 60 newest reviews and two older pages. These are individual reports, not Willow’s policy, and experiences vary.

NexLife, for comparison, is rated 4.8 out of 5 on Trustpilot, from 407 reviews (read November 1, 2026).

Canceling

How do you cancel Willow, and can you get a refund?

You cancel by messaging its care team before your next processing date. A refund depends on whether your prescription has gone to the pharmacy.

  • How: message Willow’s Patient Care Team. Its help center says you can cancel any time, with no long-term commitment, and its terms also mention canceling from your account page.
  • When: at least 2 business days before your next processing date, its FAQ says; its help center says two full calendar days. Requests after that apply to the following cycle.
  • Renewals: your plan renews automatically each month until you cancel.
  • Refunds: a full refund, in 5 to 10 business days, if you cancel before your prescription is sent to the pharmacy. Once it’s compounded, Willow says it can’t be returned, canceled or refunded.
  • If you’re not prescribed: Willow declines the order and returns your payment, its terms say.
  • Lost or late packages: Willow says it sorts it out with the pharmacy. If a package is returned, or delayed long enough to risk unsafe temperatures, it sends a replacement.

From Willow’s FAQ, cancellation policy, refund policy and terms of service (last updated August 14, 2026), read November 1, 2026.

At NexLife, plans renew until you cancel, by email, with 30 days’ notice from the day we receive it. NexLife’s plan page for compounded tirzepatide, read November 1, 2026, says: “Any authorization charge is fully refunded if you’re not prescribed or don’t proceed.”

In their words

What does the FDA say to look for in a program like Willow?

Willow says U.S.-licensed physicians prescribe and a state-licensed compounding pharmacy prepares your medication; the FDA’s advice for any program:

“Patients should obtain a prescription from their doctor and fill the prescription at a state-licensed pharmacy.”

U.S. Food and Drug Administration, FDA’s Concerns with Unapproved GLP-1 Drugs Used for Weight Loss

Willow says it replaces a package delayed long enough to risk unsafe temperatures; the FDA on a shot that arrives warm:

“The agency recommends patients not use any injectable GLP-1 drug that arrives warm or with insufficient refrigeration, as this can affect the drug’s quality.”

Who it fits

Who does Willow fit, and who may pay less elsewhere?

It depends on your medicine, your dose and how you like to pay. Here’s where Willow stands out, and where the same medicine costs less.

Willow may fit you if

  • You want a doctor-led online program with tablets or shots, a same-day assessment and unlimited messaging with your doctor, as Willow describes it.
  • You want to stop on short notice. Willow’s cutoff is 2 business days before your next processing date; NexLife asks for 30 days’ notice.

You may pay less elsewhere if

  • You take tirzepatide. Willow charges $399 to $549 a month. Month to month, it’s $225 at Cora, $129 at NexLife and $219 at Ondra Health, at every dose.
  • You’d commit to a year. Tirzepatide is $135 a month on Cora’s 12-month plan, $115 on NexLife’s and $142 on Ondra Health’s. Willow lists monthly prices only.
  • You prefer tablets. At NexLife, tirzepatide tablets are $199 a month on the 12-month plan or $229 month to month, and semaglutide tablets $165 on the 12-month plan or $199 month to month. At Willow they’re $399 to $459, and $299.
  • Your dose is going up. At Cora and NexLife the price stays the same at every dose, and Ondra Health says the same within a plan.

At NexLife

Switching from Willow, step by step

Time it to your processing date. Willow needs 2 business days’ notice to skip the next charge, and a compounded order can’t be refunded.

  1. Cancel with Willow

Message its Patient Care Team at least 2 business days before your next processing date, and keep the confirmation.

  2. Note your medicine and dose

Your medicine, whether it’s a tablet or a shot, and your dose in milligrams. A photo of the label helps.

  3. Start online, about 3 minutes

A licensed provider reviews your history and your current dose online and decides whether to continue it.

  4. Know your price

Compounded tirzepatide is $115 a month on the 12-month plan or $129 month to month, at every dose. Paying less at another licensed provider? Our Price Match Guarantee† matches your regular monthly price, with a receipt from the last 60 days. See the terms.

Four steps, from canceling with Willow to your new price.

Tablets to shots, or shots to tablets? That’s a new prescription, and your provider decides the form and the dose.

One number from then on. Questions about your order come to us, 24/7, at (949) 818-8000, and we follow up with the pharmacy for you.

Vial math

How many units is your tirzepatide or semaglutide dose?

On a compounded vial, the units depend on its strength. Here are the label doses at example strengths, with the math to check your own.

Tirzepatide

Tirzepatide doses on Zepbound’s label, with the volume and units on a U-100 syringe at an example strength of 25 mg per mL; your vial’s label decides
DosemL at 25 mg/mLUnits (U-100)On the label
2.5 mg0.1 mL10 unitsStarting dose, for 4 weeks
5 mg0.2 mL20 unitsA maintenance dose
7.5 mg0.3 mL30 unitsA step up
10 mg0.4 mL40 unitsA maintenance dose
12.5 mg0.5 mL50 unitsA step up
15 mg0.6 mL60 unitsThe maximum, and a maintenance dose

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 25 mg/mL, 10 mg is 10 ÷ 25 = 0.4 mL, or 40 units.

Semaglutide

Semaglutide dose steps from the Wegovy label, with the volume and units on a U-100 syringe at two example strengths, 2.5 mg/mL and 5 mg/mL
WeeksWeekly doseAt 2.5 mg/mLAt 5 mg/mL
1 to 40.25 mg0.1 mL (10 units)0.05 mL (5 units)
5 to 80.5 mg0.2 mL (20 units)0.1 mL (10 units)
9 to 121 mg0.4 mL (40 units)0.2 mL (20 units)
13 to 161.7 mg0.68 mL (68 units)0.34 mL (34 units)
17 onward2.4 mg0.96 mL (96 units)0.48 mL (48 units)

The math: mL = dose ÷ strength (mg per mL); units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

25 mg per mL is an example only: it’s the strength of the 15 mg multi-dose vial on Zepbound’s label (15 mg per 0.6 mL). Read the strength on your own vial’s label; your provider sets your dose, in steps at least 4 weeks apart. Every step: the tirzepatide dosage chart. Or enter your own vial’s strength in the tirzepatide and semaglutide dosage calculator. Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

You aren’t charged for medication.
Can I cancel?
Yes, by email. It takes effect 30 days after we receive it. Plans renew until you cancel; our cancellation policy has the details.
Do I need insurance?
No. Pay with an HSA or FSA card, or with Affirm, Klarna or Afterpay at checkout.

How much is Willow weight loss per month?

$299 to $549 a month, by medicine and dose (checked November 1, 2026). Compounded semaglutide is $299 to $399 and tirzepatide shots $399 to $549, with doctor access and shipping included. The pages we read list monthly prices only. At NexLife, compounded tirzepatide is $115 a month on the 12-month plan or $129 month to month, at every dose.

Is Willow Health legit?

Willow is a real, operating telehealth company: Willow Health Services, Inc., of Houston, Texas. U.S.-licensed physicians from an affiliated medical practice prescribe, and its terms name its six partner pharmacies. It’s rated 4.1 out of 5 on Trustpilot, from 382 reviews (checked November 1, 2026), and its own pages say its compounded medicines aren’t FDA-approved.

Does Willow charge more at higher doses?

For tirzepatide, yes. Its price list puts tirzepatide shots at $399 a month at the three lowest doses and $549 at 17 mg; tablets are $399 or $459 (checked November 1, 2026). Semaglutide shots are $299 up to 3.5 mg and $399 at 7 mg. At NexLife, the price is the same at every dose.

What medications does Willow prescribe?

Compounded semaglutide and tirzepatide, as weekly shots or daily tablets that dissolve under the tongue, plus a tablet that combines the two. It also offers NAD+, an injection that isn’t a GLP-1. A Willow doctor decides whether any of them is right for you.

Are Willow’s semaglutide tablets FDA-approved?

No. Willow’s own help center says “There is no FDA-approved oral-dissolving formulation of semaglutide or tirzepatide.” The FDA says it doesn’t review compounded drugs for safety, effectiveness or quality before they’re sold. The FDA-approved GLP-1 pills for weight are the Wegovy pill and Foundayo, and their labels say to swallow them whole.

Can I use HSA or FSA with Willow?

Yes. Willow says you can use an HSA or FSA card at checkout, and that all its treatments are HSA- and FSA-eligible. It doesn’t bill insurance, but you can submit receipts yourself. NexLife accepts HSA and FSA cards too, and provides a letter of medical necessity.

How do I cancel Willow?

Message Willow’s Patient Care Team at least 2 business days before your next processing date; later requests apply to the next cycle. You get a full refund only if you cancel before your prescription is sent to the pharmacy (checked November 1, 2026). Plans renew automatically until you cancel.

What do Willow reviews say?

Willow is rated 4.1 out of 5 on Trustpilot, from 382 reviews (checked November 1, 2026). In reviews we read November 1, 2026, its newest mostly praise quick replies from doctors and staff, and on-time delivery. Its low ratings, most of them older, most often say the medication didn’t seem to work, or describe disputed charges and shipping problems.

Can I switch from Willow to NexLife?

Yes. Cancel with Willow at least 2 business days before your next processing date, then start a 3-minute assessment with your current dose in milligrams. A licensed provider reviews it and decides whether to continue it. Paying less elsewhere for compounded tirzepatide or semaglutide? Our Price Match Guarantee† matches your regular monthly price.

Who can join Willow, and in which states?

Adults 18 and over who aren’t pregnant or breastfeeding, in the 37 states Willow lists, and it can’t accept people with diabetes (read November 1, 2026). NexLife is available only in states where its affiliated providers are licensed.

How fast does Willow ship?

Willow says a physician usually reviews your online assessment within 24 hours, and most patients get medication within 7 days of approval, with free shipping. At NexLife, the partner pharmacy usually ships within 1 to 2 business days, also free.

Which pharmacies does Willow use?

Willow’s terms name 6: Red Rock Home Pharmacy and Red Rock Pharmacy (Utah); Epiq Scripts and Empower Pharmacy (Texas); Absolute Pharmacy and The Pharmacy Hub (Florida). The same terms call Willow a technology platform, not a healthcare provider or pharmacy (read November 1, 2026).

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
