Will tirzepatide get cheaper? What’s changed in 2026 and what’s scheduled for 2027

Medically reviewed by the NexLife clinical team, led by Adam Kennah, MD · Updated

Tirzepatide already got cheaper: Zepbound fell to $299–$449 a month at LillyDirect in December 2025, with no new cut announced as of October 22, 2026. Compounded tirzepatide at NexLife is $139 a month on the 12-month plan, or $169 month to month, the same at every dose.

Key takeaways

  • Zepbound got cheaper on December 1, 2025. LillyDirect now charges $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg to 15 mg if you refill within 45 days. No new cut has been announced.
  • TrumpRx lists the same starting price. TrumpRx.gov shows Zepbound starting at $299, as LillyDirect does, and sends vial orders to LillyDirect. Under Medicare’s GLP-1 Bridge, people who qualify pay $50 a month through December 31, 2027.
  • December 31, 2026 is a date to watch. Foundayo’s reduced 2.5 mg and 5.5 mg prices, Wegovy’s $199 starter fills and Lilly’s Zepbound savings cards all run through then, their terms say.
  • 2027’s announced changes are mostly for semaglutide. Wegovy’s list price falls to $675 and Medicare’s negotiated semaglutide price starts on January 1, 2027. Tirzepatide isn’t on Medicare’s negotiation lists for 2027 or 2028.
  • At NexLife, compounded tirzepatide is $139 a month on the 12-month plan, or $169 month to month, the same at every dose, with no membership fee. It isn’t Zepbound, and it isn’t FDA-approved.

Timeline

What changed, and when? GLP-1 prices since November 2025

Since November 2025, Zepbound’s and Wegovy’s self-pay prices have both dropped. Novo cut Wegovy from $499 to $349 a month on November 17, 2025, and Lilly cut Zepbound vials to $299–$449 on December 1, 2025. Medicare’s $50 GLP-1 Bridge began July 1, 2026. Each dated change is below, with its source at the end of the page.

  1. The White House announces deals with Lilly and Novo Nordisk. Zepbound and orforglipron, if approved, are to average $346 a month through TrumpRx, down from $1,086. Medicare’s price for Zepbound, Mounjaro, Wegovy and Ozempic: $245, with a $50 copay.

  2. Novo Nordisk cuts Wegovy’s self-pay price from $499 to $349 a month, and offers new patients their first two fills at $199.

  3. Lilly cuts Zepbound vials on LillyDirect: 2.5 mg from $349 to $299, 5 mg from $499 to $399, and 7.5 mg to 15 mg from $499 to $449 with on-time refills.

  4. The Wegovy pill, approved December 22, 2025, goes on sale at $149 a month for the starting dose.

  5. The Zepbound KwikPen arrives at the same self-pay prices as the vials, starting at $299.

  6. Novo Nordisk says the list price of Wegovy, Ozempic and Rybelsus will fall to $675 a month on January 1, 2027, about 50% off Wegovy’s list price at the time.

  7. The FDA approves Foundayo, Lilly’s daily GLP-1 pill. Self-pay starts at $149 a month for the lowest dose.

  8. CMS says its BALANCE Model won’t start in Medicare in 2027, and extends the Medicare GLP-1 Bridge through 2027.

  9. Medicare’s GLP-1 Bridge begins: $50 a month for the Zepbound KwikPen, Wegovy or Foundayo, for people with Part D who qualify.

Prices are the makers’ self-pay prices for the medication alone. Your prescriber’s visit is extra. For today’s Zepbound prices dose by dose, and a year’s total, see Zepbound cost without insurance.

TrumpRx

What do Zepbound, Wegovy and Foundayo cost on TrumpRx?

The same as buying from the maker. On TrumpRx.gov, checked October 22, 2026, Zepbound starts at $299, the Wegovy pen at $199 for a new patient’s first two fills and the Wegovy pill at $149. Foundayo shows “up to 77% off,” by dose. TrumpRx lists each maker’s self-pay price, then sends you to the maker or gives you a coupon.

Is TrumpRx cheaper than LillyDirect? Not at the starting dose: both list Zepbound at $299, and TrumpRx sends vial orders to LillyDirect.

Have insurance or Medicare? TrumpRx prices are for paying yourself. Its Wegovy terms say you can’t bill that purchase to insurance or count it toward your deductible or out-of-pocket limit. On Medicare Part D, the $50 Bridge may cost less: see who qualifies.

What’s scheduled

What’s scheduled for late 2026 and 2027?

Three dates matter most. On December 31, 2026, Foundayo’s reduced prices, Wegovy’s $199 starter fills and Lilly’s Zepbound savings cards end. On January 1, 2027, Wegovy’s list price falls to $675 and Medicare’s semaglutide price starts. On December 31, 2027, the Medicare GLP-1 Bridge is set to end. We list only dated, sourced changes.

  1. Last day for Foundayo’s reduced prices: $149 at 2.5 mg and $199 at 5.5 mg. TrumpRx.gov lists their regular prices as $199 and $299.

  2. Last day for Wegovy’s $199 price on a new patient’s first two fills at 0.25 or 0.5 mg. After that, Novo’s pen price is $349, or $399 for Wegovy HD 7.2 mg.

  3. Lilly’s Zepbound savings cards expire, their terms say. That includes the card that brings the KwikPen’s self-pay prices to retail pharmacies. LillyDirect’s own self-pay prices list no end date.

  4. Novo Nordisk’s list price for Wegovy, Ozempic and Rybelsus falls to $675 a month. Novo says its self-pay prices don’t change.

  5. Medicare’s negotiated price for semaglutide (Ozempic, Rybelsus and Wegovy) starts: $274 for a 30-day supply, against a 2024 list price of $959.

  6. Lilly plans to submit retatrutide to the FDA. It isn’t approved, and Lilly says it can’t legally be sold.

  7. An FDA exclusivity for tirzepatide as a new chemical entity ends, the Orange Book shows. Its main patent is listed through May 13, 2036.

  8. Medicare’s GLP-1 Bridge is set to end. CMS calls bringing its BALANCE Model to Part D a “potential” next step; no 2028 plan is announced.

Not announced, as of October 22, 2026

  • A new Zepbound price for 2027, at LillyDirect or on TrumpRx.gov.
  • A Medicare negotiated price for tirzepatide. It isn’t on CMS’s lists for 2027 or 2028.
  • A tirzepatide generic. Its listed patents run through May 13, 2036 and later.
  • U.S. prices for retatrutide or CagriSema. Neither is FDA-approved.
  • Medicare coverage for weight loss after December 31, 2027.

New drugs

Will retatrutide or CagriSema push prices down?

No one can say yet. Neither is FDA-approved, so neither has a U.S. price. Novo expects a CagriSema decision by late 2026, and Lilly plans to submit retatrutide in the first quarter of 2027. The newest approved pills, the Wegovy pill and Foundayo, start at $149 a month self-pay. Every pipeline date: new weight loss drugs 2027.

  • Retatrutide, from Lilly

An investigational weekly shot. Lilly said on July 23, 2026 that it plans to submit it to the FDA in the first quarter of 2027. Until it’s approved, it has no price and can’t legally be sold. Retatrutide, explained.

  • CagriSema, from Novo Nordisk

Submitted to the FDA on December 18, 2025. On February 23, 2026, Novo said it expects a decision by late 2026. It isn’t on the FDA’s list of 2026 novel drug approvals, checked October 22, 2026.

  • Two pills, already here

The Wegovy pill went on sale January 5, 2026, and Foundayo was approved April 1, 2026. Both start at $149 a month self-pay for the lowest dose, below Zepbound’s $299. Foundayo’s cost by dose.

  • No compounded versions

The FDA says retatrutide and cagrilintide, one of CagriSema’s two ingredients, can’t be used in compounding under federal law.

Not announced: what either new drug will cost, or whether either will change Zepbound’s price. So we don’t guess.

List price

Will a list price cut lower what you pay?

Only if your costs follow list price. Novo cuts Wegovy’s list price to $675 on January 1, 2027, which can lower coinsurance or a deductible figured on list price. A flat copay stays the same, and Novo says self-pay prices don’t change. Paying cash? Novo’s cut won’t lower your price. More: Wegovy price in 2027.

  • List price

What the maker charges wholesalers. Lilly gives Zepbound’s as $499 to $1,086.37 per fill. Novo’s 2027 cut to $675 is a list-price cut.

  • Self-pay price

What you pay in cash, straight from the maker: $299 to $449 a month for Zepbound at LillyDirect. Novo says its list-price cut doesn’t change its self-pay prices.

  • Your share with insurance

Your copay or coinsurance. Novo says a lower list price helps most where costs follow list price, as with high-deductible plans or coinsurance.

  • Net price

What a payer ends up paying after rebates. In Medicare’s Bridge, the makers agreed to a $245 net price for a month’s supply, KFF reports, while patients pay $50.

Price changes: your questions

When will tirzepatide be cheaper?

Tirzepatide already got cheaper: Zepbound fell to $299–$449 a month at LillyDirect in December 2025, with no new cut announced as of October 22, 2026. Next dated changes: Lilly’s Zepbound savings cards and Foundayo’s and Wegovy’s temporary prices end December 31, 2026. Compounded tirzepatide at NexLife is $139 a month on the 12-month plan, or $169 month to month.

Will Zepbound get cheaper in 2027?

No cut is announced: as of October 22, 2026, Lilly hasn’t announced a new Zepbound price for 2027. LillyDirect charges $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg to 15 mg with refills within 45 days. Lilly’s savings cards, including the KwikPen card for retail pharmacies, expire December 31, 2026, their terms say.

What price does TrumpRx.gov list for Zepbound?

TrumpRx.gov lists Zepbound vials and the KwikPen starting at $299 a month, LillyDirect’s price for 2.5 mg, and Lilly fills vial orders through LillyDirect (checked October 22, 2026). The White House’s November 6, 2025 deal said Zepbound would average $346 a month through TrumpRx, down from $1,086.

Is TrumpRx cheaper than LillyDirect?

Not at the starting dose: TrumpRx.gov lists Zepbound at $299, the same as LillyDirect, and sends vial orders to LillyDirect (checked October 22, 2026). These are self-pay prices: the terms say you can’t bill them to insurance or count them toward a deductible.

When does the $149 Foundayo price end?

For the 2.5 mg dose, after December 31, 2026: Lilly says that price is temporarily reduced through then, and TrumpRx.gov lists the regular price as $199. The 5.5 mg dose’s $199 is temporary too; its regular price is $299. The 0.8 mg starting dose is also $149, and LillyDirect doesn’t mark that price as temporary (checked October 22, 2026).

Will compounded tirzepatide prices change?

They can, program by program: our GLP-1 price index logs every re-check with its date. Today compounded tirzepatide costs $139 to $399 a month online, all in (median $237 across 14 programs, checked October 15, 2026). At NexLife it’s $139 a month on the 12-month plan or $169 month to month, the same at every dose. Compounded tirzepatide isn’t FDA-approved.

Will Medicare negotiate a lower price for tirzepatide?

No, not so far: tirzepatide isn’t on Medicare’s drug price negotiation lists for 2027 or 2028, CMS’s lists show. Semaglutide is: its negotiated price, $274 for a 30-day supply, starts January 1, 2027. Separately, under the GLP-1 Bridge, makers agreed to a $245 net price for a month’s supply, and people with Part D who qualify pay $50 a month.

When does tirzepatide’s patent expire?

The FDA’s Orange Book lists Lilly’s main tirzepatide patent, on the drug itself, through May 13, 2036 (checked October 22, 2026). Other listed patents run to July 22, 2039 for Zepbound and December 30, 2041 for Mounjaro. A separate FDA exclusivity for tirzepatide as a new chemical entity ends May 13, 2027.

Does Novo’s 2027 list price cut lower what I pay for Wegovy?

Not if you pay cash: Novo Nordisk says the $675 list price, starting January 1, 2027, doesn’t change its self-pay prices. It mainly helps people whose costs are tied to list price, such as those with high-deductible plans or coinsurance, Novo says. Today the Wegovy pen is $349 a month self-pay, or $399 for Wegovy HD 7.2 mg.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
