Tirzepatide maintenance dose: what to expect once you reach your goal
Medically reviewed by the NexLife clinical team, led by Adam Kennah, MD · Updated
A tirzepatide maintenance dose is 5 mg, 10 mg or 15 mg once a week, per Zepbound’s label (read October 22, 2026); your provider picks yours. At NexLife, compounded tirzepatide is $139 a month on the 12-month plan, or $169 month to month, at every dose.

Key takeaways
- The label lists three maintenance doses. For Zepbound, they’re 5 mg, 10 mg or 15 mg once a week; 2.5 mg is only for starting. Your provider weighs how you respond and how you feel on it.
- Staying on held the weight off. In SURMOUNT-MAINTAIN, after 60 weeks at 10 or 15 mg, weight held steady for another year on the full dose. It rose 7.0% after stepping down to 5 mg, and 15.2% on placebo.
- Stopping brought weight back. In SURMOUNT-4, weight rose 14.0% in the year after people switched to placebo; those who stayed on lost another 5.5%. Both trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
- Priced by dose? A low dose can cost less. Fifty 410’s 3-month programs run from $133 a month at its starter tier to $183 at its high-dose tier (checked October 21, 2026). A flat price doesn’t change with your dose.
- At NexLife, maintenance costs the same at every dose: $139 a month on the 12-month plan, or $169 month to month. A year on the 12-month plan is $1,668, at 5 mg or 15 mg.
The label
What does the label say about maintenance doses?
Zepbound’s label (revised August 2026) lists three maintenance doses: 5 mg, 10 mg and 15 mg, once a week. Your provider picks one by how you respond and how you tolerate it. The 2.5 mg you started on is only a starting dose, not one to stay on. For sleep apnea, the maintenance doses are 10 or 15 mg.
| Weekly dose | What the Zepbound label says | A maintenance dose? |
|---|---|---|
| 2.5 mgweeks 1 to 4 | The starting dose, for 4 weeks. It’s for starting treatment, not for maintenance. | No |
| 5 mg | The lowest maintenance dose. It’s for weight, not for sleep apnea. | Yes, for weight |
| 7.5 mg | A step up, after at least 4 weeks at 5 mg. | No, a step |
| 10 mg | A maintenance dose, for weight and for sleep apnea. | Yes |
| 12.5 mg | A step up, after at least 4 weeks at 10 mg. | No, a step |
| 15 mgthe maximum | A maintenance dose, and the most the label allows. | Yes |

How your provider chooses. The label says to weigh how well you respond and how you tolerate the dose. If a maintenance dose doesn’t agree with you, it says to consider a lower one.
On compounded tirzepatide, your prescription sets your dose and your schedule. How many units that is depends on your vial’s strength: see our tirzepatide dosage chart.
Stalled before your goal? That’s a plateau, which is a different question: see tirzepatide plateau.
The 2026 trial
Can a lower dose keep weight off? What SURMOUNT-MAINTAIN found
Most of it, on average. In SURMOUNT-MAINTAIN (The Lancet, 2026), people who had lost weight on 10 or 15 mg held steady for another year on that dose. Weight rose 7.0% after stepping down to 5 mg, and 15.2% on placebo. The trial studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
60 weeks at the full dose
441 adults took tirzepatide at the highest dose they tolerated, 10 or 15 mg.
Three groups for a year
378 who had lost at least 5% were split at random into three groups for 52 weeks. They stayed on that dose (140 people), stepped down to 5 mg (144) or switched to placebo (94).
A safety net
Everyone had diet and activity counseling. From week 84, anyone who regained more than half their loss could restart tirzepatide.
| Group, after 60 weeks at 10 or 15 mg | Week 112, vs starting weight | Weight change in the maintenance year | Kept 80% or more of their loss* | Restarted tirzepatide (rescue) |
|---|---|---|---|---|
| Stayed on their dose140 people | −21.9% | −0.2% (held steady) | 77.5%, about 3 in 4 | 8% (11 of 138) |
| Stepped down to 5 mg144 people | −16.6% | +7.0% | 42.4%, about 2 in 5 | 25% (35 of 142) |

SURMOUNT-MAINTAIN (Horn et al., The Lancet, 2026): the trial’s main results, counting everyone randomized. *Among people whose weight had leveled off by week 60. This trial studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
What it means. On average, the full dose held the weight off. 5 mg held most of it, and placebo much less. Results on 5 mg varied from person to person, the authors note.
Fewer stomach side effects on 5 mg. In the maintenance year, nausea, vomiting or diarrhea affected 14% of people on the full dose, 7% on 5 mg and 3% on placebo.
Seen 22.4% and 17.0% elsewhere? Those figures, in Lilly’s press release, estimate what would have happened if everyone had stayed on their assigned treatment. On that basis, the three groups were 22.4%, 17.0% and 10.1% below their starting weight.
Stopping
SURMOUNT-4: what happened when people stopped?
Much of the weight came back. In SURMOUNT-4 (JAMA, 2024), people switched to placebo after 36 weeks on Zepbound saw their weight rise 14.0% over the next year, on average, despite diet and activity counseling. Those who stayed on it lost another 5.5%. The trial studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
| Group, after 36 weeks on Zepbound | Weight change, week 36 to week 88 |
|---|---|
| Switched to placebo335 people | +14.0% |
| Kept taking Zepbound335 people | −5.5% |

Study 4 (SURMOUNT-4) in the Zepbound prescribing information, and the trial’s paper in JAMA (2024). 783 people started Zepbound, stepping up to 10 or 15 mg. After 36 weeks, the 670 who went on had lost 20.9% on average, and were randomly assigned to keep taking it or switch to placebo for 52 weeks. This trial studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
The same pattern in SURMOUNT-MAINTAIN. On placebo, weight rose 15.2% in a year, and 2 in 3 people regained enough to restart tirzepatide.
Neither trial tested stopping after a few months. Both switched people to placebo only after 36 or 60 weeks of treatment.
An average isn’t a forecast for you. Some people keep more weight off than others. Your provider can help you plan, whatever you decide.
Not studied
Lower doses and longer gaps: what’s not studied?
Going below 5 mg, dosing every other week and microdosing. The Zepbound label (revised August 2026) lists none of them for maintenance, and neither SURMOUNT-MAINTAIN nor SURMOUNT-4 tested them. Without a trial, no one knows how well they hold weight off. If you want a lower dose, ask your provider about 5 mg.
| Maintenance plan | The Zepbound label | The trials on this page |
|---|---|---|
| Stay at 10 or 15 mg | A maintenance dose | Held weight off, on average, in both trials |
| Step down to 5 mg | A maintenance dose, for weight | Held most of it, on average (SURMOUNT-MAINTAIN) |
| Go down to 2.5 mg | A starting dose only | Not tested for maintenance |
| Every other week | Not in the label: every dose is weekly | Not tested |
| Microdosing | Not in the label | Not studied |
| Stop | No stopping schedule | Weight came back, on average, in both trials |

From the Zepbound prescribing information (revised August 2026), SURMOUNT-MAINTAIN and SURMOUNT-4. These trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
Why “not studied” matters. Without a trial, there’s no good evidence on how well a lower dose or a longer gap holds your weight, or how it feels.
Want a lower dose? 5 mg once a week is the label’s lowest maintenance dose, and the one SURMOUNT-MAINTAIN tested. If side effects are the reason, tell your provider: the label says to consider a lower maintenance dose when one isn’t tolerated. More on tirzepatide side effects.
Cost
What does maintenance cost per year?
It depends on whether the price follows your dose. At LillyDirect, Zepbound at 5 mg is $399 per 4 weeks, or $5,187 a year (checked October 21, 2026). Fifty 410’s compounded starter tier is $1,596 a year. A flat price costs the same at 5 mg as at 15 mg, so stepping down changes nothing there.
| Program | How it’s priced | Per month | A year |
|---|---|---|---|
| Fifty 410, starter tiercompounded, 3-month programs | By dose tier | $133 | $1,596 |
| NexLife, 12-month plancompounded | One price at every dose | $139 | $1,668 |
| Fifty 410, mid-dose tiercompounded, 3-month programs | By dose tier | $166 | $1,996 |

Prices as each company lists them: Fifty 410 and LillyDirect checked October 21, 2026, NexLife October 22, 2026. NexLife’s and Fifty 410’s prices include the provider and shipping; LillyDirect’s is for the medication alone. A year is our math: four 3-month programs for Fifty 410 ($399, $499 or $549 each), 13 four-week supplies for Zepbound, 12 months for NexLife. Zepbound’s $449 needs a refill within 45 days; otherwise it’s $699 at 10 to 15 mg. Compounded tirzepatide isn’t FDA-approved; Zepbound is. If insurance covers Zepbound for you, it may cost less.
Staying at 10 or 15 mg, or stepping down to 5 mg? At a flat price, it costs the same: $1,668 a year on NexLife’s 12-month plan either way. At LillyDirect, stepping down saves $650 a year.
Where a price set by dose wins. If your maintenance dose falls in a program’s lowest tier, you can pay less there than at a flat price, ours included. Fifty 410’s starter tier, $133 a month, costs less than any NexLife plan. Our Price Match Guarantee† matches regular month-to-month prices, not prepaid multi-month programs like Fifty 410’s. Its mid-dose ($166) and high-dose ($183) tiers cost more than our 3-month plan ($159).
Where a flat price wins. It doesn’t change if your provider moves your dose up or down, now or later. Who offers one: flat-rate tirzepatide.
Every dose, priced: tirzepatide price by dose. The lowest prices we track: most affordable compounded tirzepatide.
With your provider
How do you stop taking tirzepatide?
With a plan you make with your provider. The Zepbound label (revised August 2026) sets no taper or stopping schedule. Stepping down to 5 mg is the middle path SURMOUNT-MAINTAIN tested. Talk through why you want to stop and what you’ll watch, so the plan fits you.
Say why you want to stop
Cost, side effects or reaching your goal each lead to a different plan. Pregnant? The label says to stop Zepbound once a pregnancy is recognized, so tell your provider right away.
Ask about the middle path
Stepping down to 5 mg is the option SURMOUNT-MAINTAIN tested with Zepbound. Your provider decides whether it fits you.
Agree on what you’ll watch
Settle with your provider how you’ll track your weight, and when to check back in.
Know how restarting works
After a long break, your provider may restart you at a lower dose. Here’s how that works: restarting tirzepatide after a break.
Keep the habits either way. In both trials, people had diet and activity counseling throughout. On placebo, much of the weight still came back, on average. These trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
Do you have to stay on it forever? Not necessarily. But Zepbound is approved to reduce excess weight and keep it off long term, and the SURMOUNT-MAINTAIN authors concluded that long-term treatment is often needed.

Tirzepatide maintenance: your questions
What is the maintenance dose of tirzepatide?
Zepbound’s label lists 5 mg, 10 mg and 15 mg of tirzepatide once a week as maintenance doses; your provider picks yours. 2.5 mg is only a starting dose, and 15 mg is the most the label allows. For sleep apnea, the maintenance doses are 10 or 15 mg.
Can I go down to 5 mg after I reach my goal?
Possibly: 5 mg is one of the label’s maintenance doses for weight, and your provider decides. In SURMOUNT-MAINTAIN, people stepped down to 5 mg after 60 weeks on 10 or 15 mg. Their weight rose 7.0% over the next year, on average, while it held steady on the full dose. This trial studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
Can I take tirzepatide every other week?
Not by the label: every Zepbound dose is taken once a week, and no trial on this page tested every-other-week dosing. If you want a lower dose, 5 mg once a week is the label’s lowest maintenance dose, and the one SURMOUNT-MAINTAIN tested. Your provider decides what fits you.
Can I microdose tirzepatide for maintenance?
Microdosing isn’t studied: the Zepbound label has no microdose, and no trial on this page tested one. The label’s lowest dose, 2.5 mg, is only for starting treatment. If you want a lower dose, ask your provider about 5 mg, the label’s lowest maintenance dose.
What happens if I stop tirzepatide after reaching my goal?
On average, much of the weight comes back. In SURMOUNT-4, weight rose 14.0% in the year after people switched to placebo, while those who stayed on Zepbound lost another 5.5%. In SURMOUNT-MAINTAIN, weight rose 15.2% in a year on placebo. These trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
What happens if I stop tirzepatide after 3 months?
No trial on this page tested that: SURMOUNT-4 and SURMOUNT-MAINTAIN switched people to placebo only after 36 or 60 weeks. In both, much of the lost weight came back within a year, on average. These trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied.
Do I have to take tirzepatide forever?
Not necessarily, but it’s meant for the long term: Zepbound is approved to reduce excess weight and keep it off long term. In trials, much of the weight came back after people stopped, and the SURMOUNT-MAINTAIN authors concluded that long-term treatment is often needed. These trials studied FDA-approved Zepbound; compounded tirzepatide wasn’t studied. Your provider can help you weigh staying on, stepping down or stopping.
How much does maintenance cost at NexLife?
Compounded tirzepatide at NexLife is $139 a month on the 12-month plan, or $169 month to month, at every dose. That’s $1,668 or $2,028 a year, at 5 mg or 15 mg, with no membership fee. Priced by dose, a low dose can cost less: Fifty 410’s starter tier is $133 a month on its 3-month programs (checked October 21, 2026).
Important safety information
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).
Read the full safety information
In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
