What to expect.
Tirzepatide starts at 2.5 mg and steps up every 4 weeks or more, so the first months are about adjusting. In the 72-week SURMOUNT-1 trial of Zepbound, average weight loss was 15.0% to 20.9% depending on dose, and 9 in 10 people on 15 mg lost at least 5%. People who stopped after 36 weeks regained 14.0% over the next year. At NexLife, compounded tirzepatide is $169 a month at every dose, matched if you pay less†.

The timeline
From the Zepbound prescribing information. Your own pace depends on your dose, your body and your habits.
Getting started. 2.5 mg a week, a starting dose that isn’t meant for the long term. It takes about 4 weeks for blood levels to settle on a dose. Stomach side effects are most likely while the dose is going up.
Stepping up. The dose can rise by 2.5 mg after at least 4 weeks at each step. The earliest 15 mg can start is week 21. Your provider sets the pace; some people stay longer at a dose.
At your maintenance dose. In Study 1, people spent at least 52 weeks on their maintenance dose (5, 10 or 15 mg) before results were measured at week 72.
The trial endpoint. In Study 1, average loss was 15.0% on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg, versus 3.1% on placebo.
What Study 4 found. People who took it for 36 weeks and were then switched to placebo regained 14.0% of their weight over the next 52 weeks; those who kept taking it lost another 5.5%.

What changes the pace
Higher maintenance doses led to more weight loss on average: 15.0% on 5 mg vs 20.9% on 15 mg.
People with type 2 diabetes lost less on average: 12.8% on 10 mg and 14.7% on 15 mg in Study 2.
Stopping after 36 weeks led to regaining 14.0% over the next year in Study 4. Continuing led to more loss.
Every trial group had diet and activity advice. Protein and strength training help protect muscle as weight comes off.

A licensed provider decides if it’s right for you.
Blood levels settle after about 4 weeks on a dose, and doses step up every 4 weeks or more. In the trials, weight loss built over months; results were measured at 72 weeks.
The trials reported results at 36 and 72 weeks, not month by month. Averaged over 72 weeks, the 15 mg group’s 20.9% works out to about 0.3% of body weight a week; real loss isn’t that even.
At 3 months you’re typically still stepping up the dose. The label reports results at 36 and 72 weeks; your provider can tell you what your progress means.
In Study 1, 56.7% of people on 15 mg lost at least 20% of their weight by week 72, versus 3.1% on placebo.
In Study 4, people switched to placebo after 36 weeks regained 14.0% of their weight over the next year on average, while people who continued lost another 5.5%.
Compounded tirzepatide hasn’t been studied in clinical trials and isn’t FDA-approved. These results come from trials of Zepbound.
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).
Read the full safety informationIn studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.
Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
